Overview


If you have an exceptionally long questionnaire in a spreadsheet, it may be beneficial to utilize the data import functionality. But first you'll need to add the information to a ZenGRC template formatted for proper import.

After successfully importing a questionnaire, you can still use the questionnaire builder. So knowing your way around the builder will enable faster and easier edits.

Downloading the Questionnaire Templates

  1. Click Tools | Data Import.
  2. Select templates in the following order:
    1. Questionnaire Templates
    2. Questionnaire Template Questions
    3. Questionnaire Template Question Options
    4. Questionnaire Template Question Conditions

By creating the import template in this manner, you are able to build all parts of a questionnaire at one time.

  1. Questionnaire Templates - This contains the questionnaire name, along with the code, which is used to connect this questionnaire to the Questionnaire Template Questions template.
  2. Questionnaire Template Questions - This is where individual questions for your questionnaire reside. These include the following columns:
    1. Questionnaire Template -  Add the code from the first template to this column.
    2. Code - This is the unique identifier for each question that will be used in the next template. 
    3. Question Type - The information in this column is for whether the question is yes/no, file upload, radio button, etc. and must be formatted as follows:
      1. file_upload
      2. radio
      3. rich_text
      4. checkboxes
      5. date
      6. datetime
      7. email
      8. url
      9. additional_text
      10. text_field
  3. Questionnaire Template Question OptionsThis is where answers to questions are added and include the following columns:
    1. Questionnaire Template Question - Put the code from the question in the previous template here.
    2. Title - Each row is a single selection to a question. For example, if a question has a yes/no response, then each of those selections would be placed on a separate row. The title of one row would say "Yes" and the title of a separate row would say "No."
  4. Questionnaire Template Question ConditionsCreate the conditionals for the options to the questions. The columns for this template include the following:
    1. Shown By Question Option - This is where the code from the third template is placed. It assigns the condition to the question's option.  
    2. Questionnaire Template QuestionAdd the code of the question from the second template that corresponds to the conditional question.

Reviewing Responses


When a recipient returns a questionnaire, the object on which the questionnaire was sent displays the responses. For example, if you sent a questionnaire requesting information on a specific program in ZenGRC, the recipient's responses then reside in that program. After reviewing the answers, utilize the object's Comments section to capture notes. This helps others see pertinent information without having to click into the questionnaire.
